Insurance companies entering Origin of Health Coverage code A (SHOP) or B (Employer-sponsored coverage) on Part I of Form 1095-B must also complete Part II, Information about Certain Employer-Sponsored Coverage. Employers reporting self-insured group health plan coverage on Form 1095-B should enter Origin of Health Coverage code B on Part I and not complete Part II.
Sponsor Management Dialog
The Sponsor Management dialog, shown above, manages employer-sponsored coverage information required by insurance companies for completing Part II of Form 1095-B. Sponsor Management can be accessed from the File menu of Form 1095-B or by clicking the sponsor management button located in the Sponsor ID field of Form 1095-B (shown below).
The Sponsor ID is a unique alphanumeric identifier assigned to each employer sponsoring health coverage. Insurance companies entering Origin of Health Coverage codes A or B must enter a Sponsor ID. Employers reporting self-insured group health plan coverage should leave the Sponsor ID field blank and enter an Origin of Health Coverage code B.
The button bar displayed along the bottom of the dialog provides functions for navigating, inserting, deleting, editing, and importing sponsor records. Position your mouse over each button to display its function.
For those interested in importing sponsor records, a schema layout is included Appendix B. The Import Mapping Utility can be used to map a spreadsheet of sponsor records to this schema. Sample import files can be found on the Help menu. |
